RAG in Production: Why Enterprise Knowledge Assistants Fail—and How to Fix Them

A convincing RAG demo can be built quickly. A trustworthy enterprise knowledge system requires disciplined content, retrieval, evaluation, security, and operations.

RAG is a system, not a feature

Retrieval-augmented generation combines content ingestion, indexing, search, context construction, generation, and user experience. Weakness in any layer can create an answer that sounds confident but is incomplete, outdated, or unauthorized.

Teams often tune prompts when the real problem is missing content, poor chunking, weak metadata, or retrieval that optimized similarity instead of usefulness.

Fix the knowledge foundation

Assign content owners and define authoritative sources. Preserve useful structure, dates, versions, product or region metadata, and access labels during ingestion. Create update and deletion processes so the index reflects the source of truth.

Chunking should match how users ask questions and how evidence must be cited. One universal chunk size rarely works for policies, manuals, tables, and long reports.

Improve retrieval quality

Combine semantic and keyword retrieval when appropriate, apply metadata filters, rerank candidates, and avoid flooding the model with marginally relevant text. Query rewriting and decomposition can help complex questions, but each transformation must be evaluated.

Respect source permissions at retrieval time. Filtering after content reaches the model is too late.

Evaluate with real questions

Build a representative evaluation set with answerable, ambiguous, unanswerable, and adversarial questions. Measure retrieval recall, source relevance, groundedness, answer completeness, citation accuracy, and refusal behavior. Review results by content domain to find systematic gaps.

Operate for continuous trust

Monitor latency, failures, cost, unanswered questions, user feedback, and content freshness. Version prompts, models, indexes, and evaluation results. Use uncertain or high-impact interactions to trigger clarification, source-first responses, or human escalation.
